- GENERAL INFORMATION FOLLOWED BY A TENTATIVE STATEMENT OF WORK NOTICE: This is not a solicitation but rather a Request for Information (RFI) (Sources Sought) to determine capability of potential sources and is for information and planning purposes only. Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify capable firms for Research Brain Bank Removal and Disposal SVCS at the Coatesville VAMC. Responses must be submitted by 4:00 PM (EST) October 23, 2019. Submit responses to the information requested in this Sources Sought notice via email to William.Schlobohm2@va.gov. All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this sources sought notice for market research purposes, and shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. Suggested PSC: AT81 (R&D Other Transportation: Transporting Hazardous Materials) Suggested NAICS: 562211 (Hazardous Waste Treatment and Disposal) Open to suggestions from the market as to a better NAICS and/or PSC. TENTATIVE STATEMENT OF WORK BRAIN AND 10% FORMALIN REMOVAL COATESVILLE VAMC SCOPE: The contractor shall provide all management, labor, supplies, transportation, and equipment to remove roughly 320 brains in 1 gal buckets with 10% formalin from the second story of building 11. All contractor services shall be performed in a manner that ensures the health and general well-being of patients, staff, and visitors, while remaining in compliance with State, local, and Federal environmental regulations. Any and all applicable Environmental Protection Agency (EPA) regulations and rules shall be followed (www.epa.gov). All Federal, state, and local laws will be strictly followed. GENERAL REQUIREMENTS: Contractor will remove containers from second floor storage room via the stairwell. Contractor will remove the containers, brains, and 10% formalin solution from the Coatesville VAMC campus. All Federal, state, and local laws for hazardous waste will be strictly followed. Contractor will provide the Green Environmental Management System (GEMS) Program Manager with the total weight of formalin disposed of. Contractor will provide the Green Environmental Management System (GEMS) Program Manager with the total weight of biological material disposed of. Contractor will dispose of the brains as biological waste. All Federal, state, and local laws for biological waste will be strictly followed. Contractor will provide the Green Environmental Management System (GEMS) Program Manager with a manifest showing that it was properly disposed of. Contractor will dispose of the 10% formalin as hazardous waste. All Federal, state, and local laws for hazardous waste will be strictly followed. Contractor will provide the Green Environmental Management System (GEMS) Program Manager with a legal manifest showing that it was properly disposed of. The Contractor will provide a container identification labeling system as a measure to enable the VAMC to verify accuracy of the Contractor s weight disposal report. All loading will be performed by the Contractor or his/her employees without any assistance from VA employees. In the event climate or weather conditions become unsuitable for work or may induce an environmental hazard, then the entire work shall be rescheduled to a date and time satisfactory to the Contractor and Environmental Management Services. Postponement will not reflect any penalties assessed to the Contractor, nor should there be any additional cost to the government. All services provided are to be completed by a certified, trained and licensed hazardous waste/biological waste handler/transporter in any and all categories as required by the State of Pennsylvania. Contractor will provide licenses and permits, which indicate compliance with EPA and respective state environmental regulatory agency regulations pertaining to transportation, collection, incineration, and disposal. Contractor shall provide the work schedule of when services will be performed and submit to Environmental Management Services and GEMS Program Manager no later than the contract start date. Contractor shall conduct the removal activities during normal business hours 0800-1630 Monday through Friday excluding Federal Holidays unless other arrangements are made in advance through the COR. Contractors are urged and expected to inspect the site and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after the contract has been awarded. All site visits must be arranged with the COR. In the performance of this contract, the Contractor shall take such safety precautions as the COR or facility Technical POC or designee may determine to COR or facility Technical POC designee will notify the Contractor of any noncompliance of the foregoing provision and action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions in which attention has been directed. Such notice when served on the Contractor or his/her representative at the site of the work shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or any part of the work and hold the Contractor in default. Contractor shall provide their own spill kit and spill team in the event that any hazardous or biological waste is released in the building or outside during transportation and/or separation. All Federal, state, and local laws will be strictly followed. The Contractor will be responsible for immediate cleaning, repackaging, and disinfecting of spill areas. Contractor will immediately notify the fire department x4285 and the GEMS Program Manager x2114 in the event of any spill, regardless of amount spilled. LOCATION OF SERVICES TO BE PROVIDED Department of Veterans Affairs (Main Facility); 1400 Blackhorse Hill Rd. Coatesville, PA 19320. Building 11, second floor (no elevator) VOLUME OF MATERIAL 320 one gal buckets containing biological material and 10% formalin solution 7-10 thirty gal bags of biological material contained in a 17-cu ft freezer Various chemicals hazardous and non-hazardous amounting to one or two 55-gal drums WORKING CONDITIONS Materials to be removed are in a two-story building with: No elevator, one inside stairwell, two outside stairwells No water No heat or air conditioning
